A 12-inning thriller between the Pittsburgh Pirates and the Chicago Cubs was decided on a disastrous error by Pirates’ outfielder Gregory Polanco, who slipped and fell on his face trying to track a routine fly ball. The Cubs had the bases loaded, so the winning run scored easily.
